In order to be very clear and upfront with our patients, Dr. Edward Alvarez wants to clarify his practice policy on all refunds. As you have been made aware, the science and art of Aesthetic Medicine, and Medical Spa Treatments involves significant risks of complications. These complications, including scarring, poor healing, infection, etc. (described to you in detail on the consent forms), occasionally can, and do, result in a poor outcome and may in fact be responsible for you possibly looking WORSE from your procedure rather than better. Although Dr. Edward Alvarez and designated team members have significant experience in Aesthetic Medicine, injecting, and medical spa treatments, he and they cannot, and do not in any way guarantee any type of treatment results, or that your procedure will not result in a poor outcome. This can, and does occur, despite performance of the procedure in an appropriate manner. Because of this significant risk, NO REFUNDS will be offered if you suffer any complications resulting in a poor outcome, or an outcome other than what you were hoping for or expecting. In fact, if it is necessary to repair the poor outcome of a procedure, you may incur significant additional charges.
If for any reason you are not satisfied with a product or in the event that there is an allergic reaction to the product you purchased from us, you may return it within 30 days of the original purchase date with a valid receipt for a full refund if you purchased the product after consulting with one of our skin care specialists.
All spa service sales are final. If there is a valid medical reason as to why you cannot utilize a Medical Spa Service or Spa Service Package that you purchased, we will allow you to use your remaining balance towards other services of equal value. The only time that we will provide a refund for your purchase of a Medical Spa service or package, is if your complaint is accompanied by a practicing physician's note explaining your medically-inclined inability to utilize the service within 6 months of the purchase date of said service, at which point we will present you with a 50% refund of the original service purchase price.
Due to high demand of service, all appointment no shows as well as same day cancellations, scheduled for any of our providers require a $100 service fee, per hour of scheduled time, that will be charged. All cancellations must be done 24 hours prior to the scheduled appointment time in order to avoid this fee. We understand that health issues may arise, if this is to happen, we require that you provide us with a Doctors/Hospitalization note as to exactly what is occurring. No further appointments or treatments will be provided until this fee is paid in full.
